摘要:目的 通過血管回聲跟蹤(echo-tracking,ET)技術(shù)對(duì)頸動(dòng)脈血管彈性的定量檢測(cè),探討年齡及飛行時(shí)間對(duì)男性飛行員頸動(dòng)脈血管彈性的影響. 方法 根據(jù)年齡將72例飛行員分為20~29歲(27人)、30~39歲(30人)和40~49歲(15人)3組.應(yīng)用ET技術(shù)檢測(cè)各組飛行員的頸動(dòng)脈血管,并采用t檢驗(yàn)分別對(duì)各個(gè)年齡組頸動(dòng)脈血管彈性值[僵硬度(stiffness parameter,β)、彈性模量(pressure-strain elastic modulus,Ep)、順應(yīng)性(arterial compliance,AC)和脈搏波傳導(dǎo)速度(pulse wave velocity calculated from β value,PWVβ)]與國(guó)人正常測(cè)量值(對(duì)照組)進(jìn)行對(duì)比分析,采用方差分析對(duì)各組間血管彈性值和飛行時(shí)間進(jìn)行比較,通過直線相關(guān)和偏相關(guān)分析對(duì)各組飛行員年齡與飛行時(shí)間對(duì)頸動(dòng)脈血管彈性的影響進(jìn)行分析. 結(jié)果 飛行員各組β、Ep及PWVβ均較國(guó)人正常測(cè)量值降低,AC增高.飛行員20~29歲及30~39歲組血管彈性值與其對(duì)照組比較,有統(tǒng)計(jì)學(xué)意義(t=2.061~6.712,P<0.05或P<0.01),40~49歲組與對(duì)照組比較,無統(tǒng)計(jì)學(xué)意義.通過直線相關(guān)分析,30~39歲組的年齡、飛行時(shí)間與β、Ep和PWVβ均存在顯著正相關(guān);在20~29歲和30~39歲組中,年齡與飛行時(shí)間存在顯著性正相關(guān)(r=0.655、0.723,P=0.001、0.000).通過偏相關(guān)分析,在控制飛行時(shí)間因素時(shí),30~39歲組中β和Ep值與年齡存在顯著正相關(guān)(r=0.516、0.452,P=0.007、0.020),即隨著年齡的增加,β和Ep值增加. 結(jié)論 年齡對(duì)男性飛行員頸動(dòng)脈血管彈性影響較大,而飛行時(shí)間影響較小.ET技術(shù)對(duì)飛行員頸動(dòng)脈血管彈性值的定量檢測(cè)有助于進(jìn)一步了解男性飛行員的血管功能狀態(tài).
